4-復學生/延修生/選課申請表(Application form for readmitted students/extended students/course selection)

申請時間(Application time)：依課務組公告。(According to the announcement of Curriculum Section)

　延修生/復學生寒暑假欲選修未額滿課程時使用辦理。(Extended and readmitted students can use this form to apply for courses that are not full during the summer and winter breaks.)

申請流程(Application process)：

　1.自行填寫加選課程資料(限未額滿課程)－2.開課單位登記並核章－

3.學生所屬系所主管核章－4.課務組線上加課並存查－

1.Fill in the course information for adding and selecting courses (limited to courses that are not full) on your own.

2.The offering department registers and seals the form.

3.The student's department supervisor seals the form.

4.The Office of Academic Affairs adds the course online and saves the record.

填妥本表單後，如有學分費疑慮者，應加會出納組辦理。(After completing this form, if there are any doubts about the credit fees, please contact the Cashier's Office.)
